Well... I finished it last week and I don't remember seeing any unusual characters in the ending.

Perhaps you took some shortcuts the first time you finished it, and just didn't get to see some of the more "rare" enemies, such as the Big Boo, the ghost Lakitu or the Wrigglers?

There is a secret test level that can only be accessed using a game genie. As far as secret characters, I have done a bit of rom hacking with Super Mario World, and I never saw anything that was not in the game.

It's pretty boring. Just a Donut Plains 1-like area, with the word "TEST" written in turn blocks in the sky.

The enemy your probably thinking of is Torpedo Ted. I used to think that was a unused character for a while since he only appears in one level.

I have a proto of SMW, and it's the same as the released version, according to dumps. It came from a Nintendo Game Play counsellor, so yes, it's real.

Most of those screenshots are the same magazine scans that we've all seen a thousand times, sadly. They didn't come from any dumped cartridge or anything. If you can run that page through a translator, I'm sure it'll tell you the same thing, I'm too loopy to do it myself.

The others are on the released cart and are accessible.

Yes, I think it was the level below the Cookie Bridge, the one that leads to Star World 3. Not sure though.
