Even Amazon and many eBay auctions are showing this cover, but I haven't found any evidence that this design got ever printed on a box.

Could it be a prototype cover? Any ideas on where did this come from and why has it spread so broadly?

I've never actually played the game, but I've never seen that first cover that you posted, only the second (though I may well have only seen it in magazines). I'd say it might be a Europe/North America difference (I'm in the U.S.), but both have the oval seal of quality instead of the circle. Here's an odd column A/column B mix from eBay:

Now that's odd... But it could be a pirate/custom box, it's kind of fishy how he put that extremely high price just to disappear from eBay.

For the record, as far as I remember, Maru's Mission was never released in Europe. The copies I played back then were US imports.
